Important Plan Information
Program Guidelines and Limitations
The San Pablo Smiles Savings Plan is a dental discount plan, not dental insurance.
The plan is secondary to any other dental plan and is subject to the following guidelines and limitations.
Program Guidelines
- The benefit coverage table is subject to annual revision.
- The savings plan remains active for 12 months from the purchase date.
- The plan must be paid in full at enrollment.
- The membership is nonrefundable.
- Refunds will not be issued when a participant does not use the plan’s benefits.
The Plan Cannot Be Used
- In conjunction with another dental plan
- For treatment that falls outside the treating dentist’s professional capabilities
- For referrals to specialists
- For hospitalization or hospital charges
- For dental care covered under an automotive claim
