Membership Dental Savings Plan
The San Pablo Smiles Savings Plan is designed to provide affordability and greater access to quality dental care. With your savings plan, there are no deductibles, no claim forms, no pre-authorization requirements, no pre-existing condition limitations and no waiting periods. Becoming a member will allow savings on EVERY treatment and procedure.
This program is a discount plan, not a dental insurance plan and is secondary to any other dental plan. It cannot be used:
- In conjunction with any other dental plan
- For treatment which, in the sole opinion of the treating dentist, lies outside the realm of their capability
- For referrals to specialists
- For hospitalization or hospital charges of any kind
- For cost of dental care that is covered under automotive claims
Program Guidelines:
- Benefit coverage table is subject to annual revision
- Savings plan is good for 12 months from date of purchase
- Plan must be paid in full up front
- Non-refundable – no refunds will be issued to participants that do not utilize dental plan
